Penerapan Polyurethane Foam Padding untuk Pencegahan Pressure Injury Wajah pada Operasi BESS

Abstract

Pressure injury merupakan komplikasi perioperatif yang dapat terjadi akibat tekanan berkepanjangan, gaya geser, dan gangguan perfusi jaringan selama tindakan pembedahan. Pasien yang menjalani Biportal Endoscopic Spine Surgery (BESS) dengan posisi prone berisiko mengalami pressure injury pada area wajah karena dahi, pipi, hidung, dan dagu menerima tekanan kontinu dari headrest ketika pasien berada dalam anestesi umum dan tidak mampu mengubah posisi secara mandiri. Studi kasus ini bertujuan menggambarkan penerapan polyurethane foam padding sebagai pressure-redistributing support surface tambahan untuk mendukung pencegahan pressure injury wajah pada pasien yang menjalani BESS di Rumah Sakit X. Studi melibatkan tiga pasien BESS dalam posisi prone. Pengkajian risiko menggunakan Risk Assessment Scale for the Development of Injuries due to Surgical Positioning (ELPO), disertai observasi kondisi kulit sebelum dan sesudah operasi. Intervensi keperawatan perioperatif meliputi pengkajian risiko, safe positioning, penggunaan headrest standar, pemantauan integritas kulit, dan penambahan polyurethane foam padding pada dua pasien; satu pasien menggunakan headrest standar tanpa padding tambahan. Seluruh pasien memiliki skor ELPO kategori risiko rendah, yaitu 15–17, tetapi memiliki faktor risiko prosedural yang sama berupa posisi prone, anestesi umum, dan durasi operasi lebih dari dua jam. Pasien tanpa padding tambahan mengalami eritema ringan sementara pada area wajah tanpa memenuhi kriteria pressure injury stage 1, sedangkan dua pasien yang menggunakan polyurethane foam padding tetap memiliki kulit utuh tanpa eritema maupun pressure injury. Temuan ini menunjukkan bahwa polyurethane foam padding yang dipadukan dengan pengkajian risiko dan pemantauan kulit terstruktur berpotensi membantu mempertahankan integritas kulit wajah selama operasi BESS.
