Penerapan Manajemen Cairan pada Pasien DHF dengan Risiko Hipovolemia di RS X Tangerang Selatan

Dengue Hemorrhagic Fever merupakan infeksi virus dengue yang dapat meningkatkan permeabilitas kapiler, menyebabkan kebocoran plasma, penurunan volume intravaskular, dan risiko hipovolemia. Studi kasus ini bertujuan menggambarkan penerapan manajemen cairan pada pasien dengan Dengue Hemorrhagic Fever yang mengalami risiko hipovolemia melalui perbandingan kondisi klinis dua pasien. Desain yang digunakan adalah studi kasus deskriptif dengan pendekatan proses keperawatan pada dua pasien di ruang rawat inap RS X Tangerang Selatan pada Juni 2026. Data diperoleh melalui wawancara, observasi, pemeriksaan fisik, dokumentasi rekam medis, pemeriksaan laboratorium, serta pemantauan asupan dan keluaran cairan. Intervensi mengacu pada Standar Intervensi Keperawatan Indonesia dan dilaksanakan selama tiga kali dua puluh empat jam. Hasil menunjukkan bahwa pasien pertama mengalami tekanan darah 90/70 mmHg, nadi 102 kali/menit, mukosa kering, turgor menurun, dan hematokrit 40,8%, sehingga pemantauan difokuskan pada stabilitas hemodinamik dan volume intravaskular. Pasien kedua memiliki hemodinamik relatif stabil, tetapi mengalami stomatitis yang menghambat asupan oral, hematokrit 44,4%, serta trombositopenia berat hingga 25.000/mm³ disertai petekie. Setelah intervensi, kondisi klinis dan status hidrasi kedua pasien membaik, dengan keseimbangan cairan positif sebesar +937,5 mL dan +1.105 mL. Kadar trombosit saat pemulangan meningkat menjadi 57.000/mm³ dan 41.000/mm³. Manajemen cairan yang disertai pemantauan hemodinamik, hidrasi, keseimbangan cairan, dan laboratorium membantu mempertahankan volume sirkulasi serta mencegah perburukan hipovolemia. Pendekatan perlu disesuaikan dengan karakteristik klinis setiap pasien.
